La cannula lunga: un’alternativa alla cannula corta periferica? Una revisione della letteratura per conoscere vantaggi e svantaggi del loro utilizzo

Abstract

INTRODUZIONE: La cannula corta periferica è il dispositivo più comunemente utilizzato per gli accessi venosi a breve termine, ma nei pazienti con difficult intravenous access (DIVA) può risultare inadeguata, associandosi a un maggior rischio di fallimento del dispositivo, complicanze, ripetuti tentativi di cannulazione e ritardi nell’avvio della terapia. Una possibile alternativa è rappresentata dalle cannule periferiche lunghe che, pur avendo un costo unitario superiore, potrebbero offrire vantaggi clinici e assistenziali.

OBIETTIVI: Valutare l’efficacia delle cannule periferiche lunghe rispetto alle cannule corte in termini di durata del dispositivo, affidabilità e comfort del paziente.

METODI: È stata condotta una revisione narrativa strutturata della letteratura consultando le banche dati PubMed e Cochrane Library. Sono stati inclusi studi pubblicati tra il 2019 e il 2024, in lingua italiana o inglese e disponibili in full text.

RISULTATI: Sono stati inclusi 19 studi. Le cannule lunghe hanno mostrato una permanenza media compresa tra 8 e 14,5 giorni, rispetto alle 55 ore delle cannule corte. Il numero medio di tentativi di cannulazione è risultato inferiore per le cannule lunghe (1,39 vs 2,85) e gli studi hanno evidenziato una minore incidenza di fallimento del dispositivo e di complicanze rispetto alle cannule corte.

CONCLUSIONI: Le evidenze disponibili suggeriscono che le cannule periferiche lunghe, soprattutto se posizionate con guida ecografica, rappresentino una valida alternativa alle cannule corte nei pazienti DIVA, garantendo una maggiore durata del dispositivo, una riduzione dei tentativi di cannulazione e delle complicanze, con potenziali benefici per il comfort del paziente.
