Mental Health Triage in the Emergency Room: literature review and development of an algorithm

BACKGROUND:
During the last decades there has been a considerable increase in the incidence of psychiatric disorders worldwide along with an increased need to improve both inpatient and community-based nursing care, given the significant negative impact that these conditions have on patients' quality of life. Very frequently people suffering from acute psychiatric disorders go to emergency rooms (ER) to receive immediate nursing care and support, so it’s essential for the triage nurse to be able to evaluate the patients with appropriate and specific assessment tools.
AIM:
The aim of this study was to examine the literature searching for specific assessment tools for the mental health triage (MHT), afterwards it’s been developed an evidence-based assessment tool together with an algorithm, both suitable for the Italian healthcare system.
METHODS: The inclusion and exclusion criteria were established, after that was conducted a literature research in the databases of PubMed, CINAHL, Scopus, Embase, PsycInfo and Cochrane Library; 16 articles were selected for the qualitative review.
RESULTS:
All the studies included in the narrative review emphasized the importance of the use of specific tools for the MHT, since they ensure an objective and standardized evaluation and the assignment of the correct level of priority to the patients; this allows patients to avoid long waiting times and overcrowding of the ER, thus limiting the patients’ distress.
CONCLUSIONS: Since the study’ results showed the benefits for both patients and nurse staff related to the use of specific assessment tools, it’s recommended their use in all the healthcare facilities.
