Abstract

BACKGROUND:
Difficult vascular access occurs in 30% of patients requiring vascular access. It is a clinical condition that results in multiple attempts to cannulate the vein. It is managed by a dedicated team of nurses with advanced skills in peripheral venous access (PVA), PICC and Midline implantation using ultrasound technique.

AIM:
To perform a literature review investigating outcomes and benefits of management of these patients by a nursing team with advanced skills in vascular access.

METHODS:
The PubMed, CINAHL, EMBASE, Cochrane Library, ILISI and Web of Science databases and the official websites of the INS and GAVeCeLT were consulted.

RESULTS:
Seven studies were included. The team consisted of an average of 15 nurses and 4 medical specialists. It emerges a success rate of the procedure at the first attempt of 88-100% and a reduction of 90% and 70% of the rates of CLABSI and catheter-related infections. On average 44.1% of the devices were removed by the end of the treatment. Waiting time for a suitable device was reduced by 80%. The average satisfaction rate was 88.23% (average score recorded 8.56/10) and an average of 2.85/10 for perceived pain. Companies reported an average reduction of 261,358.27 euros/year in business expenses.

CONCLUSIONS:
A dedicated team for difficult vascular access represents a fundamental resource for the delivery of quality care through timely management, reduction of complication rates, optimization of care timing and expenses.
