Sustainability assessment in operating rooms: content and face validation of the green theatre checklist

Abstract

BACKGROUND:
This study describes the process of translation, adaptation, and validation of the Italian version of the Green Theatre Checklist (GTC), a tool designed to improve environmental sustainability in operating rooms. The aim is to reduce the environmental impact of the healthcare sector, which contributes to 5% of global carbon emissions, with a particular focus on operating rooms, one of the most impactful areas. The checklist was developed by the Royal College of Surgeons of Edinburgh to provide practical guidelines on how to minimize carbon emissions during surgical procedures.

METHODS:
The validation process took place in Italy between June - October 2024 and involved a heterogeneous sample of nurses, doctors, and academics. International guidelines for the translation and cultural adaptation of instruments were followed, with two independent translations and a comparison between versions. The validation included verifying the comprehensibility, clarity, and relevance of the checklist items.

RESULTS:
Results showed a good level of acceptability (S-CVI = 0.93), with the exception of some items that were subsequently modified to improve clarity.

CONCLUSIONS:
The Italian Green Theatre Checklist represents a useful tool to guide healthcare personnel in making surgical practices more sustainable without compromising the quality of care. This study provides an important contribution to promoting more sustainable healthcare in Italy, with the aim of reducing the environmental impact of the sector and raising awareness among operators towards more responsible practices.
